Health and Wellness

 
Marian High School provides health and wellness services for students during the school day and is directed by a registered nurse. The nurse coordinates the programs the State of Indiana requires for the prevention and detection of physical ailments. The nurse is responsible for distribution of medicine for those students under a doctor's care, and also collects and maintains state-mandated health records, immunizations, and health screenings.
Deb Cseh, R.N., is the Marian Director of Health and Wellness. Mrs. Cseh is a 1983 graduate of Marian High School. Her clinical nursing experience includes Cardiac Rehabilitation, Interventional Radiology, Obstetrics and Gynecology.  Mrs. Cseh also worked as a School Nurse for the South Bend School Corporation.

The Health and Wellness Office is open from 8:00am until 3:00pm during regularly scheduled school days and is located across the hall from the cafeteria.

Students with non-emergency issues should go to the Nurse’s Office during break or lunch so that class time is not missed.

A student absence should be reported to the Attendance Office.

Marian High School is required to have on file specific health information for all its students. Students who are new to Marian High School must submit these documents as part of the application process. Please refer to the Admissions portal for the link to upload and submit these forms.

STUDENTS WHO NEED TO TAKE MEDICATION

Marian High School is concerned for student health and safety. Part of overseeing student safety includes direct supervision of medications being consumed while students are at school. Supervision includes both prescribed and over-the-counter medications.

Students who use inhalers, must carry an Epi-Pen for allergic reactions, or are insulin-dependent diabetics must have a completed Consent for Administration of Medication form in addition to any forms associated with their condition on file in the Health and Wellness Office.

Consent for Administration of Medication

Students who must take any form of medication during school hours must do so under the following conditions.

  • Students who need prescription or over-the-counter medications administered at school must bring the medication directly to the Attendance Office.
  • All the medications must be in the original container.
  • Prescription medication must have a pharmacy label indicating the student’s name, type of medication, dosage, and times to administer.
  • Over-the-counter medication (Advil, Tylenol, or Aleve, for example) must be in the original container and labeled with the student’s name.
  • A permission form signed by both the prescribing physician and a parent/guardian is required for prescription medications.
  • A permission form must be completed and signed by parent/guardian for administering over the counter medications.
  • Medication will be administered under the direct supervision of the school nurse, principal, assistant principal, dean of students, main office secretary, or attendance office secretary.
  • No student, under any circumstances, is allowed to consume or administer medication in school without the direct supervision of the school nurse, principal, assistant principal, dean of students, main office secretary, or attendance office secretary.
  • Students need a pass/admit before or after receiving medication.
Students should know that their Epi-pen will be kept in the Main Office so that they are immediately accessible when medically indicated and are carefully monitored for expiration. An Allergies Protocol Form, an Epi-Pen Usage Form, and a Consent for Administration of Medication Form must be completed by prescribing physician and signed by the parent/guardian and the student, then submitted to the Health and Wellness Office to keep on file.
 

Students should keep inhalers on their person so they are accessible when medically indicated. An Inhaler Self Administration Form and a Consent for Administration of Medication must be completed by the prescribing physician and signed by the parent/guardian and the student, then submitted to the Health and Wellness Office to keep on file.

Consent for Administration of Medication Form

Students who have insulin-dependent diabetes must be closely monitored for metabolic imbalance. In order to better understand each diabetic student's unique medical needs and provide appropriate aid as needed, a Diabetic Profile Form and a Consent for Administration of Medication Form must be completed by the prescribing physician and signed by the parent/guardian and the student, then submitted to the Health and Wellness Office to keep on file.
 

 

STUDENT ILLNESS

If your child is not feeling well, please keep him or her home. Students must be fever free for 24 hours without the use of fever reducing medications before returning to school. If your teen has a fever or is exhibiting other symptoms such as body aches, vomiting, diarrhea, or coughing, please keep him or her at home. Students are advised to remain home if suffering from active illness in order to protect other students and staff from possible disease transmission.

Parents are requested to report all confirmed or suspected contagious diseases to the school nurse.

When a student becomes ill or injured during class, a pass to the Health and Wellness Center must be obtained from the current teacher. If a student is too ill to remain in school, the nurse will notify the parents.

Students are not allowed to make arrangements to go home on their own. If the parent is unavailable, the persons designated on the student’s emergency card will be called. It is important that all phone numbers be kept current for the protection of the student.

 

STUDENT INSURANCE COVERAGE

A portion of each student’s course fees is for student/athlete insurance coverage. Students have coverage for injuries and accidents that happen during the school day at Marian High School or during school-sponsored activities.

This coverage is a secondary policy. Eligible covered expenses will be paid which are in excess of other valid and collectible insurance. Claims should first be submitted to the parent's medical insurance company.

If a student is injured while at school, his/her parent should seek medical care for the student through their usual medical provider. Once care is initiated, the parent must contact the Marian Business Office, who will obtain information required to complete the school's portion of the claim form. The claim form will then be mailed to the student's parent, who must complete the parent/student portion of the form.

The parent must mail the completed from along with an Explanation of Benefits (EOB) along with corresponding itemized provider statement(s) to the plan administrator's address shown on the claim form. All correspondence after this point will be between the plan administrator and the student's parent.

Completed claim forms must be submitted within 90 days of the date of injury.

Injuries sustained while participating in Marian sanctioned athletic training or events will be managed through the Athletic Training Office. Student athletes with an injury that must be treated by medical professionals must report to the Marian Athletic Training Office to begin the process of insurance reporting.
